If you are a veterinary clinic interested in contacting me regarding my relief services, please note the following guidelines:

• I do require a completed service agreement prior to performing relief work at any veterinary clinic. This contract reviews my status as an independent contractor, your clinic's confidentiality, cancellation and payment policies, fee schedule, etc. The agreement is required when specific dates are requested, but may also be completed preemptively at any time to retain relief services when needed.
